I noticed an interesting and very practical book entitled, "How To Reach and Teach Children with Challenging Behavior" by Kaye Otten Ph.D. & Jodie Tuttle M.Ed., two behavior specialists who together have 40 years of experience working with students who have challening behavior in classsrooms. This book was published by John Wiley & Sons, Inc. in 2011. The book covers Functional Behavioral Assessments, Positive Behavior Support, Social Skills Instruction (group & individual), Preventing Challenging Behavior, Adapting for Weaknesses in Executive Functions, Facilitating Academic Engagement, Group & Individual Reinforcement Systems, Logical Consequences, Designing & Implementing Effective Behavior Intervention Plans, 4 case studies, Managing Dangerous Behavior, Physical Restraint & Seclusion, as well as 10 Reproducible Tools. Books on theory are great as a starting point, but this book has many user-friendly practical ideas.

Reach & Teach Children with Challenging Behavior
Another very popular book to address behavior problem at school is the book by Dr. Nancy Rappaport & Jessica Minahan entitled "The Behavior Code: A Practical Guide to Understanding and Teaching the Most Challenging Students," published in 2012. I ordered the Kindle edition to read on my iphone, but I'm planning to get the paper editions for our office. The book has a companion book of pre-made forms and handouts. The book talks about the use of functional behavior analysis, how behavior communicates a purpose if properly analyzed using the A-B-C method, how to set up adaptations, build relationships with kids, and develop effective responses to misbehaviour. Their method us called the F.A.I.R. plan. This stands for Functional hypothesis of behavior and antecedent analysis, Accommodations, Interaction Strategies, and Response Strategies. The method also recommends teaching kids coping strategies & social skills, not just stopping maladaptive behaviors. The book talks about how to apply the method to students with anxiety, oppositional behavior, withdrawal, and sexualized behavior. I've seen Dr. Rappaport speak on YouTube, and she's convinced me that the book has merit.
